ABSTRACT

Objective: To investigate the differences of immune microenvironment between stage T1N3 and stage T3N0 breast cancer patients and explore the relationship between M1 macrophage infiltration and lymph node metastasis in breast cancer. Methods: Clinical information and RNA-sequencing (RNA-Seq) expression data of stage T1N3 (n=9) and stage T3N0 (n=11) breast cancer patients were extracted from Molecular Taxonomy of Breast Cancer International Consortium (METABRIC) databases. Using CIBERSORT, the proportions of 22 types of immune cells were calculated, and then the differences of immune cell infiltration between stage T1N3 and T3N0 patients were compared. From 2011 to 2022, pathologic specimens were collected from breast cancer patients who underwent curative resection at the Cancer Hospital, Chinese Academy of Medical Sciences, including 77 at stage T1N3 and 58 at stage T3N0.The METABRIC database analysis results were verified by examining the density of M1 macrophages in tissues using dual-staining immunohistochemistry. Results: METABRIC data analysis showed M1 macrophage was the highest proportion, 15.85% in stage T1N3 breast cancer; M2 macrophage was the highest proportion, 13.07% in stage T3N0 breast cancer.M1 macrophage proportions were statistically different between patients with stage T1N3 and stage T3N0 (P=0.010). The dual-staining immunohistochemistry analysis of breast cancer tissues showed M1 macrophage density (median) of 62.0 and 38.0 cells/mm(2) for stage T1N3 and T3N0, respectively. The difference was statistically significant (P=0.002). Conclusion: The density of M1 macrophages is notably higher in stage T1N3 patients and is associated with lymph node metastasis.

ABSTRACT

OBJECTIVE@#This study aimed to investigate the effects of caprylic acid (C8:0) on lipid metabolism and inflammation, and examine the mechanisms underlying these effects in mice and cells.@*METHODS@#Fifty-six 6-week-old male C57BL/6J mice were randomly allocated to four groups fed a high-fat diet (HFD) without or with 2% C8:0, palmitic acid (C16:0) or eicosapentaenoic acid (EPA). RAW246.7 cells were randomly divided into five groups: normal, lipopolysaccharide (LPS), LPS+C8:0, LPS+EPA and LPS+cAMP. The serum lipid profiles, inflammatory biomolecules, and ABCA1 and JAK2/STAT3 mRNA and protein expression were measured.@*RESULTS@#C8:0 decreased TC and LDL-C, and increased the HDL-C/LDL-C ratio after injection of LPS. Without LPS, it decreased TC in mice ( P < 0.05). Moreover, C8:0 decreased the inflammatory response after LPS treatment in both mice and cells ( P < 0.05). Mechanistic investigations in C57BL/6J mouse aortas after injection of LPS indicated that C8:0 resulted in higher ABCA1 and JAK2/STAT3 expression than that with HFD, C16:0 and EPA, and resulted in lower TNF-α, NF-κB mRNA expression than that with HFD ( P < 0.05). In RAW 264.7 cells, C8:0 resulted in lower expression of pNF-κBP65 than that in the LPS group, and higher protein expression of ABCA1, p-JAK2 and p-STAT3 than that in the LPS and LPS+cAMP groups ( P < 0.05).@*CONCLUSION@#Our studies demonstrated that C8:0 may play an important role in lipid metabolism and the inflammatory response, and the mechanism may be associated with ABCA1 and the p-JAK2/p-STAT3 signaling pathway.

ABSTRACT

To systematically evaluate the efficacy and safety of Gongxuening Capsules in the treatment of abnormal vaginal bleeding after medical abortion. CNKI, Wanfang, SinoMed, VIP, PubMed, Cochrane Library and EMbase databases were retrieved to comprehensively collect the clinical randomized controlled trials(RCTs) of Gongxuening Capsules for treatment of abnormal vaginal bleeding after medical abortion from the establishment of the databases to October 10, 2020. Literature screening, data extraction and quality evaluation were conducted independently by two system reviewers according to the inclusion and exclusion criteria. Cochrane Handbook bias risk assessment tool was used for the literature methodology quality evaluation, RevMan 5.3 software was used for Meta-analysis, and the evidence quality of outcomes was evaluated by the evidence quality grading system(GRADE). A total of 16 RCTs were inclu-ded. The results of Meta-analysis showed that as compared with the western medicine treatment alone, the addition of Gongxuening Capsules to the western medicine treatment can reduce the amount of vaginal bleeding(RR=1.23, 95%CI[1.19, 1.27], P<0.000 01), shorten vaginal bleeding time(RR_(≤15 d number of people)=1.39, 95%CI[1.31, 1.48], P<0.000 01; MD_(number ofdays)=-1.20, 95%CI[-1.66,-0.74],P<0.000 01). However, there was no obvious advantage in abortion effect(RR=1.02, 95%CI[0.99, 1.06], P=0.14) and menstrual recovery(MD=-0.35, 95%CI[-0.96, 0.25], P=0.25). The results of GRADE showed that the grading level was low for vaginal bleeding volume and vaginal bleeding time, and extremely low for abortion effect and mens-trual recovery. In terms of safety, 16 studies reported adverse events. Only one study showed no adverse events and the rest showed transient nausea, vomiting, stomach burning, upper abdominal discomfort and other gastrointestinal symptoms. The results show that the addition of Gongxuening Capsules to the application of western medicine in treatment of drug abortion can reduce the amount of vaginal bleeding and shorten vaginal bleeding time, but the abortion effect and menstrual recovery have no obvious advantages. The use of Gongxuening Capsules helps to achieve less adverse reactions and higher safety. Due to the small sample size of the included studies and many methodological quality problems, no conclusions with clinical guidance value can be obtained. Large sample-zise, high-qua-lity randomized controlled trials are still needed for further verification.

ABSTRACT

To systemically evaluate the efficacy and safety of Maxing Shigan Decoction in the treatment of community acquired pneumonia(CAP) and provide a reference for the treatment of CAP. Databases of CNKI, Wanfang, VIP, SinoMed, EMbase, Cochrane Library, Web of Science and PubMed were searched(from inception to May 30, 2020) to screen the randomized controlled trials(RCTs) of Maxing Shigan Decoction in treating CAP. Two authors independently screened and selected relevant literature and extracted data based on the inclusion and exclusion criteria. Any disagreement or differences was resolved through discussion. The bias risk assessment tool recommended by Cochrane handbook was used to evaluate the quality of the included studies, and RevMan 5.3 software was used for data analysis. Seventeen RCTs were finally included, involving 1 309 patients. Meta-analysis showed that Maxing Shigan Decoction combined with conventional Western medicine treatment could improve clinical efficacy in patients with CAP more effectively as compared with conventional Western medicine treatment alone, mainly in terms of anti-inflammatory effects, a decrease in C-reactive protein(CRP) content(MD=-6.01, 95%CI[-10.95,-1.06], P=0.02)and white blood cell(WBC) count, a decrease in procalcitonin(PCT) level(MD=-0.74, 95%CI[-0.77,-0.71], P<0.000 1), and shortened recovery time of cough and fever. Maxing Shigan Decoction has certain curative effect on CAP, but there are problems in the methodology of included studies. High-quality stu-dies are still needed for further verification.

ABSTRACT

Objective:To explore the structural differences in series scores of Hamilton Anxiety Scale (HAMA) in Parkinson's disease patients with anxiety. Methods:From January, 2016 to December, 2017, 180 inpatients with Parkinson's disease in Center for Neurodegenerative Disease, Beijing Tiantan Hospital were reviewed. Their age, gender, course of disease were recorded, and were assessed with HAMA, Hamilton Depression Scale (HAMD), unified Parkinson's Disease Rating Scale III (UPDRSIII),Hoehn-Yahr stage and Mini-Mental State Examination (MMSE). They were divided into non-anxiety group, possible anxiety group, anxiety group and very anxiety group according to the scores of HAMA. The demographics, clinical data and series scores of HAMA were compared among these groups. The correlation between HAMA and HAMD were anlyzed. Results:The course of disease, UPDRSIII scores and the most improvement ratio by drug increased with the severities of anxiety (F > 4.951, P < 0.001), as well as the series scores of HAMA (F > 188.241, P < 0.001); however, the proportion of psychogenic anxiety score increased with the severities of anxiety. The scores of HAMA correlated with the scores of HAMD (r = 0.665, P < 0.05). Conclusion:The more serious the anxiety is, the more psychogenic are in Parkinson's disease patients.

ABSTRACT

Objective:To study the prevalence and related factors of apathy in patients with Parkinson's disease (PD). Methods:From November, 2017 to December, 2019, 254 PD patients in our hospital were included. According to Starkstein Apathy Scale (SAS), they were divided into apathy group and non-apathy group. Clinical data such as demographic data, motor symptoms, non-motor symptoms and motor complications were collected for comparison between two groups. Logistic regression analysis was performed to investigate the risk factors of apathy in PD. Results:Among 254 PD patients, 124 (48.8%) cases were in apathy. Compared with non-apathy group, apathy group was older in age and age of onset, higher in the scores of Movement Disorder Society United Parkinson's Disease Rating Scale part III (MDS-UPDRS Ⅲ), Hamilton Depression Rating Scale (HAMD), Hamilton Anxiety Rating Scale (HAMA) and Pittsburgh Sleep Quality Index (PSQI) (t > 2.291, P < 0.05), and lower in the scores of Montreal Cognitive Assessment (MoCA) and Mini-Mental State Examination (MMSE) (t > 22.424, P < 0.001). There was no statistically significant difference in gender, time of education, body mass index (BMI), disease course, Hoehn-Yahr (H-Y) stage, wearing-off phenomenon, dyskinesia, on-and-off phenomenon, and the scores of Rapid Eye Movement Sleep Behavior Disorder Screening Questionnaire (RBDQ) and Epworth Sleeping Scale (ESS) between two groups (P > 0.05). Logistic regression analysis showed that the age of disease onset, MoCA and HAMD scores were correlated with apathy in patients with PD (P < 0.05). Conclusion:The presence of apathy in PD may be associated with older age of disease onset, severity of depression and cognitive impairment.

ABSTRACT

Objective:To explore the structural differences and the trend in series scores of Hamilton Depression Scale (HAMD) in patients with Parkinson's disease (PD). Methods:From June, 2017 to March, 2018, 168 PD patients from Beijing Tiantan Hospital were reviewed. Their genders, ages, courses of disease, Hamilton Depression Scale (HAMD) scores, unified Parkinson's disease rating scale III (UPDRS III) scores,Hoehn-Yahr stage and Mini-Mental State Examination (MMSE) scores, the most improvement ratios by PD drug and other clinical data were collected. The patients were divided into non-depression group (n = 61), mild depression group (n = 68) and moderate depression group (n = 39) according to the HAMD scores. The series scores of HAMD among the groups were compared. Results:The factor scores all increased with depression exacerbating (F > 10.546, P < 0.001), except day and night change in the mild depression group, while the proportion of anxiety/somatization score decreased, and the proportion of cognitive disorder and retardant scores increased. Conclusion:The depression structure changes in PD patients with depression exacerbating, which means decrease of anxiety/somatization and increase of cognitive disorder and retardant.

ABSTRACT

Objective To analyze the reasons of misdiagnosis of primary syphilitic chancre and strengthen the understanding of atypical features of this disease. Methods A case series of twenty-seven challenging primary syphilis patients who were not immediately recognized as chancre was included in our study. The clinical data including the patients' age, sex, skin lesions, HIV status, syphilis serologic test results, treatment, and follow-up results were collected. Hematoxylin-eosin and immunohistochemistry staining of skin biopsy sections were reviewed. Results Four female cases with extragenital chancres presenting as erythema or erosive skin lesions on the nipple were misdiagnosed as Paget's disease or eczema. The disorder of missed or misdiagnosed male cases manifested as syphilitic balanitis or multiple chancres on the penis root and adjacent pubis rather than coronal sulcus or frenum. Patients with nonreactive nontreponemal tests at initial presentation were also easily missed or misdiagnosed. Conclusion Primary syphilis presenting as multiple lesions rather than a single chancre, at atypical locations, or with a nonreactive nontreponemal test result, tends to be missed or misdiagnosed.

ABSTRACT

OBJECTIVE: To explore the method of management of asymptomatic bacterial vaginosis,and to evaluate the effect of probiotics on asymptomatic bacterial vaginosis and analyze the benefit gained by those patients.METHODS: A multicenter,randomized,double-blind,controlled study was performed with probiotics in test group,and placebo in control group,from september 2013 to september 2016 in 4 hospitals.Forty five patients in test group were treated with vaginal capsule probiotics,and placebo was used in forty-five patients in control group.The follow-up in both groups lasted for 4 weeks.Cure rate,relapse rate and progression rate in both groups were analyzed to evaluate the effect and the benefits gained by patients.RESULTS: Ninty patients were recruited in the study,and seventy-nine patients finished the study and were followed up with complete information.Two weeks after the treatment,in FAS the cure rate in test group was significantly higher than that of the control(44.4% vs. 6.7%,P0.05).In PPS,the cure rate in test group was significantly higher than that of the control(52.5% vs. 2.6%,P0.05).After 2 weeks of treatment,there was no relapsed patient.Four weeks after the treatment,in FAS the cure rate in test group was significantly higher than that of the control(46.7% vs. 8.9%,P0.05).In PPS,the cure rate in test group was significantly higher than that of the control(57.5% vs. 5.1%,P0.05).Four weeks after the treatment,there was no relapsed patient,either.CONCLUSION: Vaginal capsule probiotics can be used to treat asymptomatic BV with high cure rate,less side effect and low relapse rate.

ABSTRACT

Objective To investigate the difference of hypoxia-inducible factor 1a (HIF-1a) and vascular endothelial growth factor (VEGF) expression in endometrial cancer and para-carcinoma tissues, and to explore the clinical significance of hypoxia and the two proteins in the development and progression of endometrial cancer. Methods From Jan. 2011 to Dec. 2012, 128 patients with endometrial cancer underwent surgery in Tongji Hospital of Tongji University. The expression of HIF-1a and VEGF in cancer tissues and paired para-carcinoma tissues was detected using immunohistochemical method. The patients were followed up regularly, and the relationship between the expression of HIF-1a and VEGF and the prognosis of the patients was analyzed. The hypoxic cell model of human endometrial cancer was constructed to detect the expression of HIF-1a and VEGF proteins and observe the cell proliferation, invasion and apoptosis. Results The positive rates of HIF-1a and VEGF in cancer tissues were significantly higher than those in the para-carcinoma tissues (both P0.05). In the hypoxic cell model of human endometrial cancer, the expression levels of HIF-1a and VEGF were significantly increased, cell proliferation and invasion were significantly increased, and the cell apoptosis was significantly reduced (all P<0.05). Conclusion HIF-1a and VEGF are related to the progress of endometrial cancer, and positive expression of HIF-1a indicates a poor prognosis.

ABSTRACT

Objective To explore the relationship between the polymorphisms of exon 41 in COL6A3 and sporadic isolated dystonia in China. Methods A total of 127 outpatients with isolated dystonia and other 130 age-and gender-matched healthy controls were collected their blood samples. The single nucleotide polymorphism (SNP) was screened from 1000 Genomes Project. Genotype was detected with polymerase chain reaction-restriction fragment length polymorphism and the genotype and allele distribution were compared between the patients and the controls. Results Two SNPs in exon 41 in COL6A3 were found,named rs1131296 and rs2270669.There was no difference be-tween the patients and the controls in both genotype and allele(χ2<1.829,P>0.05).There was no difference in the age of onset among the patients with various genotypes(P>0.05). Conclusion Polymorphism of exon 41 in COL6A3 gene may not contribute to risk of sporadic isolated dystonia in China.

ABSTRACT

Objective To compare the characteristics of tremor and improvement after acute levodopa challenge test in young onset Parkinson's disease(YOPD)and late onset Parkinson's disease(LOPD)with a cross-sectional study. Methods From Match to September,2017,70 Parkinson's disease inpatients with rest or postural tremor in at least one extremity were included,in which 23 patients were in YOPD group,and other 47 in LOPD group according to their ages of onset.They finished acute levodopa challenge test and were analyzed for dominant tremor frequen-cy,amplitude and rhythm under resting state,posturing and holding 1000 grams state,respectively. Results YOPD group was younger(t=-2.423,P<0.01)with lower Hoehn-Yahr stage(χ2=-4.604,P<0.05),and needed less equivalent dose of levodopa at early stage(first five years)(t=-2.119,P<0.05).There was a bigger ratio of patients with rest tremor in frequency of four to six Hz in YOPD group than in LOPD group(χ2=3.896,P<0.05). Rigidity and bradykinesia scores of LOPD group positively correlated with the course of disease (r=0.34, P<0.05),while it was not found in YOPD group. Conclusion Tremor expresses most in the classical way in YOPD patients,and tremor analysis could help to diagnose young adults. Both YOPD and LOPD patients get well in acute levodopa challenge test, while YOPD patients need less equivalent dose of levodopa at early stage.

A high performance liquid chromatography-tandem mass spectrometry (HPLC-MS/MS) method was established for the determination of nine active ingredients of Xuebijing Injection in rat plasma and the pharmacokinetic properties of Xuebijing Injection in rats were studied after intravenous admin as the mobile phase for gradient elution. A mass spectrometry was applied with electrospray ionization (ESI) ion source in the negative ion multiple reaction monitoring (MRM) mode. All pharmacokinetic parameters were processed by non-compartmental analysis with DAS 2.0 software. After intravenous administration of 6 mL·kg⁻¹ Xuebijing Injection, the main pharmacokinetic parameters of ferulic acid, benzoylpaeoniflorin, danshensu, chlorogenic acid, rosmarinic acid, hydroxy-methyl safflower yellow A, paeoniflorin, albiflorin, and oxypaeoniflorin were as follows: Cmax(85.01±22.33), (56.78±22.78), (36.05±8.77), (73.73±20.26), (173.19±67.12), (20 646.96±2 666.65), (21 416.61±6 427.14), (181.88±38.38), (770.73±230.87) μg·L⁻¹; AUC(0-t n) (801.9±112.3), (1 005.6±406.0), (714.9±237.7), (1 496.3±413.9), (2 316.1±372.2), (1 207 295.3±248 582.6), (788 781.0±308 551.0), (4 785.4±1 076.0), (33 456.2±11 946.1) min·μg·L⁻¹. This method is sensitive and accurate, applicable for the pharmacokinetic study of Xuebijing Injection, and provides a basis for clinical research of Xuebijing Injection.

ABSTRACT

Objective To describe the change of practice of ICU advanced practice nurses after attending ICU Clinical Nurse Specialist(CNS) training program in Shanghai. Methods A purposive sampling method was used to recruit 15 ICU advanced practice nurses and semi-structured interviews were conducted. Thematic analysis was used for data analysis. Results Four themes regarding practice change were extracted,including extension of scope of practice,transformation of emphasis in work,promotion of clinical reforms and improvement of self-development. Conclusion The practice of ICU advanced practice nurses has changed after training,while the degree of change varies in different themes. Further quantitative researches should be conducted to provide more information for the development of ICU advanced practice nurses.

AIM: To observe the clinical efficacy and safety of vincamine sustained release capsules on non- arteritic anterior ischemic optic neuropathy ( NAION) . · METHODS: Patients who were diagnosed with monocular onset NAION in acute stage from January to September 2015 were divided into two groups. Routine treatment such as steroid pulse therapy and neurotrophic treatment were given to all the patients. Vincamine was added to the treatment group patients with 30mg twice a day for 3mo. The best corrected visual acuity ( BCVA), mean deviation ( MD) of visual field, retinal nerve fiber layer ( RNFL ) , ganglion cell complex ( GCC ) , pattern visual evoked potential ( PVEP ) and OCT results were analyzed before and after the treatment. ·RESULTS:Totally 42 eyes of 42 patients were enrolled in our study. There were 27 patients in the treatment group, aged from 33 to 79 years old, the average value was 55. 55± 11. 83 years old. The control group has 15 patients, aged from 40 to 70 years old, the average value was 55. 71 ± 10. 06 years old. There were no statistical differences between the two groups in the baseline. After 3mo of the treatment, MD value of the two groups were lower compared with the baseline, the difference was statistically significant in the treatment and control group respectively (t= 2. 342, 2. 692; P = 0. 027, 0. 041). The difference of PVEP amplitude and potential of the two groups before and after the treatment were not statistically significant. The thickness of retinal nerve fiber layer and the ganglion cell complex were all lower than the baseline, and the difference was statistically significant (P<0. 001). The treatment of the two groups were both effective, the treatment group has better treatment effect than the control group. Adverse events related to the treatment of vincamine had not been found. ·CONCLUSION:Vincamine is helpful in the treatment of non-arteritic anterior ischemic optic neuropathy.

ABSTRACT

<p><b>OBJECTIVE</b>To assess nutritional status and define gender- and age-specific handgrip strength (HGS) cut-point values for malnutrition or nutritional risk in elderly inpatients.</p><p><b>METHODS</b>A cross-sectional study of 1,343 elderly inpatients was conducted in the Chinese PLA General Hospital. Nutrition Risk Screening (NRS 2002) and Subjective Global Assessment (SGA) were administered. Anthropometric measurements and blood biochemical indicators were obtained using standard techniques. The gender- and age-specific receiver operating characteristic (ROC) curves were constructed to evaluate the HGS for nutritional status by SGA and NRS 2002. Sensitivity, specificity, and areas under the curves (AUCs) were calculated.</p><p><b>RESULTS</b>According to NRS 2002 and SGA, 63.81% of elderly inpatients were at nutritional risk and 28.22% were malnourished. Patients with higher HGS had an independently decreased risk of malnutrition and nutritional risk. The AUCs varied between 0.670 and 0.761. According to NRS 2002, the optimal HGS cut-points were 27.5 kg (65-74 years) and 21.0 kg (75-90 years) for men and 17.0 kg (65-74 years) and 14.6 kg (75-90 years) for women. According to SGA, the optimal HGS cut-points were 24.9 kg (65-74 years) and 20.8 kg (75-90 years) for men and 15.2 kg (65-74 years) and 13.5 kg (75-90 years) for women.</p><p><b>CONCLUSION</b>Elderly inpatients had increased incidence of malnutrition or nutritional risk. HGS cut-points can be used for assessing nutritional status in elderly inpatients at hospital admission in China.</p>

ABSTRACT

The objective of this study is to develop a new-type biodegradable, biocompatible curcumin-loaded nanoerythrosomes (Cur-RBC-NPs) by means of the sonication method. The size of Cur-RBC-NPs was optimized by varying drug loading parameters. The morphology, size distribution, stability, in vitro release pattern, cellular uptake of nanoparticles and in vitro anti-tumor effects were evaluated, respectively. The results showed the prepared Cur-RBC-NPs were nearly uniform spheres, with an average diameter of (245.7 ± 1.3) nm. Encapsulation efficiency (EE) and load efficiency (LE) of Cur-RBC-NPs were 50.65% ± 1.36% and 6.27% ± 0.29%. And the nanoparticles had a good sustained release property. According to the in vitro experiment, Cur-RBC-NPs were effectively taken in by tumor cells, and exhibited a significant anti-tumor effect. In conclusion, the method for preparing Cur-RBC-NPs is convenient, with a good sustained release behavior and anti-tumor efficacy, and so expected to be a new-type nano-drug delivery system in clinical practice.

ABSTRACT

The international cooperated research projects of the Human Microbiome Project (HMP) and Metagenomics of The Human Intestinal Tract (MetaHIT) were officially launched in 2007, which indicated the era of metagenomics research of microorganisms in human gastrointestinal tract had been coming. Each human body is a superorganism which is composed of 90% commensal microorganisms, especially the intestinal microorganisms. The intestinal microorganisms play an important role on health maintenance since they are involved in the absorption and metabolism of nutrients in the human bodies. Herein, we review the research progress in the mechanism of intestinal microorganisms in human diseases. Our purpose is to provide novel ideas on human health and therapeutic targets of diseases.
